Partner Spotlight: Healthy Homes Coalition Of West Michigan

What programs do you offer for young children and their families?
Ready by Five is a family-centered, collaborative case management program aimed at reducing home environmental hazards through education, supplies and referrals. Lead Hazard Control helps homeowners protect their children from lead poisoning with a HUD-based grant that remediates lead hazards in homes built before 1978. Third Ward Lead Reduction Initiative provides water kits for anyone living in the 3rd Ward of The City of Grand Rapids. These families will receive water filtering supplies, education on lead, and an informal in-home risk assessment of their pipes and fixtures.

How do families enroll in your programs?
Families can enroll in our programs by calling (616) 241-3300 to talk with our Intake Specialist who will check their eligibility and begin enrollment with a five minute home hazard screening.

How does your organization use parent voice to inform your programs?
We have monthly Community Ambassador Conversations to spotlight the issues and challenges that parents/caregivers are experiencing in the community we serve and influence the focus of our programming. Ready by Five also requests client/caregiver feedback at the beginning, mid-way, and end of programming to order to inform any areas for improvement.

What do you most appreciate about your GSC membership?
We are thankful for the support and resources that GSC provides us with along with opportunities to connect with families who are in need of our services. GSC is an exceptional community partner, through the partnership we have reached new community members.
